Proactive Discharge Planning




Length of Program: 90 minutes


Course Description:

This course is designed to assist senior management staff, nursing leaders, case management personnel, and social workers in assessing their organization’s compliance with Medicare Conditions of Participation regarding discharge planning requirements. Nationwide there has been an increase in HCFA (or state) onsite visits to acute care facilities. Many of the visits have been prompted by complaints related to inadequate discharge planning. Don’t let your facility be one of those chosen!

Target Audience:

The course is designed for registered nurses, chief nursing officers, case managers, nursing managers, social workers, utilization managers, and front-line nursing staff responsible for incorporating case management activities in the nursing process.


At the conclusion of this program, participants will be able to:

  • Know the benefits and advantages of proactive discharge planning.

  • Know how to assess your facility’s discharge planning process and understand whether it meets Medicare’s Condition of Participation requirements.

  • Know the differences between the roles of the RN Case Manager vs. Social Worker as it relates to discharge planning.

  • Know about documentation requirements.

  • Know about the overall value gained through effective, regularly scheduled interdisciplinary patient care conferences.
